Polypharmacy can lead to many complications in the older adult population like increased side effects, non-adherence, and functional and cognitive decline. Pharmacists are a key player in intervening in this patient population to optimize their health outcomes. This session will explore a holistic approach and strategies to manage polypharmacy . This session will also review best practices and how to overcome communication barriers with prescribers.

Review strategies on how to optimize medication management and tackle deprescribing.
Discuss how to overcome barriers to deprescribing among clinicians.
Given a polypharmacy patient case, develop a plan to deprescribe and manage the patient with an interdisciplinary team.
