As a pharmacist, you are held responsible for a lot of evidence-based quality measures. This may range from ensuring 5-star ratings for your community pharmacy to improving quality measures for your health system or encouraging patients to take risk-reducing medications to considering lower-cost medications. For some patients, “evidence-based medicine” prevents fractures, heart attacks, and hospitalizations or saves lives. For others, it is not as straight forward as pushing “send” for a statin. This presentation will discuss how to use the PRAPARE tool to assess for social determinants of health and how to use a patient-centered evidence-based medicine framework to take the whole patient into consideration.
