Social Determinants of Health play an important role in influencing chronic diseases including diabetes. This session will dive into the impact of health disparities and the role of pharmacists in diabetes management. Utilizing a patient case, screening and interventions to improve patient outcomes will be discussed. Additionally, innovative care models will be highlighted.
