Critical Connections: Understanding Social Determinants of Health

Description

Our homes, schools, workplaces, neighborhoods, and communities contribute to our overall wellness. These, among other factors, comprise the social determinants of health (SDOH). HHS emphasized the importance of SDOH by including them in its Healthy People 2020 initiative. But what do they mean for pharmacists? By understanding SDOH, you gain greater insights into your patients’ struggles and can direct them to appropriate resources. At this session, an expert in SDOH will help you do exactly that.  

Pharmacist Learning Objectives

At the completion of this application-based activity, participants will be able to:

  1. Define social determinants of health and how they can impact an individual's health and well-being.
  2. Identify the factors that influence health based on where one is born, grows, works, lives, and ages.
  3. Demonstrate how pharmacists can play a role in helping patients overcome social barriers to optimal health.
