  1. Identify potential patient care services for the community-based pharmacy practice by reviewing demographics and needs of patients in the community. 
  2. Analyze the costs to provide new services, identify payment opportunities, and structure services to establish sustainable business models. 
  3. Describe required authority for proposed services and steps required for credentialing by proposed payers. 
  4. Analyze existing practice infrastructure and resources and develop an action plan for practice transformation using case examples.
